Note that the application fee is $125. This fee will be applied to the instructor certification fee at the time of acceptance. Please note, however, that the application fee is nonrefundable, and application is not a guarantee of acceptance.

Note also that, to complete this application, you will need to submit a background check from an authorized government agency or reputable background-screening provider in your country of residence. Your background check should cover/include, at a minimum, sexual offender registries, where applicable, as well as all relevant criminal databases available within your jurisdiction, including convictions and pending cases.

Applicants in the U.S. may submit a "Complete"-level check from BackgroundChecks.com.

Your background check should be attached to this form, but should also be sent directly from the issuing agency or background check provider to RicksonGracieUnited@gmail.com.
